SPECIAL TRAITS

  • All-Around Vision. The suileach has multiple eyes on its body, allowing it to see in all directions. It can’t be surprised while it isn’t incapacitated and has advantage on saving throws against being blinded.
  • Amphibious. The suileach can breathe air and water.
  • Keen Sight. The suileach has advantage on Wisdom (Perception) checks that rely on sight.
  • Multiple Eyestalks. The suileach begins with five eyestalks. While it has more than one eyestalk, the suileach has advantage on saving throws against being blinded, charmed, deafened, frightened, stunned, or knocked unconscious. Whenever the suileach takes 40 or more damage in a single turn, one of its stalks dies. If all its stalks die, the suileach dies. At the end of its turn, it grows two eyestalks for each of its stalks that died since its last turn, unless it has taken 40 or more fire damage since its last turn. The suileach regains 20 hit points for each eyestalk regrown in this way.
  • Wakeful. While the suileach sleeps, at least one of its eyestalks is awake.

ACTIONS

  • Multiattack. The suileach makes as many Frightful Gaze attacks as it has eyestalks.
  • Frightful Gaze. Ranged Spell Attack: +8 to hit, range 30/60 ft., one creature. Hit: 10 (3d6) psychic damage, and the creature must succeed on a DC 16 Wisdom saving throw or be frightened of the suileach until the end of the suileach’s next turn.
  • Bite. Melee Weapon Attack: +12 to hit, reach 10 ft., one target. Hit: 24 (5d6 + 7) piercing damage. If the target is a Medium or smaller creature, it must succeed on a DC 15 Dexterity saving throw or be swallowed by the suileach. A swallowed creature is blinded and restrained, has total cover against attacks and other effects outside the suileach, and takes 17 (5d6) acid damage at the start of each of the suileach’s turns. If the suileach takes 30 damage or more on a single turn from a creature inside it, the suileach must succeed on a DC 16 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, which fall prone in a space within 10 feet of the suileach. If the suileach dies, a swallowed creature is no longer restrained by it and can escape from the corpse by using 20 feet of movement, exiting prone.
  • Constrict. Melee Weapon Attack: +12 to hit, reach 20 ft., one creature. Hit: 29 (4d10 + 7) bludgeoning damage, and the target is grappled (escape DC 20). Until this grapple ends, the target is restrained, and the suileach can’t constrict another target.

ABOUT

The suileach is a massive aberration of many branching eyestalks, each terminating in multiple eyes. A frothing maw is at its center.

In local legends, suileachs (SWIL-e-AK) are known as “the Many-Eyed.” The creature’s ability to regenerate makes it a formidable opponent, and its many eyes grant it a wide range of vision. A suileach may be sought out by those seeking rare materials that can be harvested from its body, but such an endeavor is fraught with peril.

Suileachs dwell in dark, remote caves and tunnels, where they hunt for prey that venture too close to their lair. The creature’s many eyes grant it unparalleled vision, allowing it to detect threats from all directions. When prey approaches its lair, a suileach bursts out of hiding and uses its eyestalks to immobilize its victim while its chomping maw devours them.
