The top of this human skull has been sawed off such that it can be open and shut like a cooking pot. The inside of the skull is black with soot and grease and gives off the scent of incense. While the skull appears as some sort of incense burner, it carries a curse.

Whenever any non-evil creature attempts to burn incense within the skull, it begins spewing noxious vapors and any non-evil creatures within a 30-foot radius must make a DC 18 Wisdom save or become filled with fury, attacking the nearest creature on their turn. Each round thereafter, affected creatures must make a new saving throw or be affected as confusion. The effect ends for a creature after 1 minute or when they successfully save.

If the incense is extinguished, all affected creatures have advantage on their saving throws to end the confusion effect. However, extinguishing the malicious skull causes the remaining vapors to turn caustic for 1d4 rounds. Creatures within the cloud must make a DC 18 Constitution save each round on their turn or take 1d6 acid damage. Those failing this save are afflicted with a hideous rash that causes them to have disadvantage on Charisma checks made to interact with humanoids (and other “civilized” creatures, at the GM’s discretion) until the rash is cured with lesser restoration or remove curse. Creatures immune to disease are immune to this effect.
