Choose a creature other than an undead or construct that you can see within range. A spectral hand reaches into that creature’s torso and appears to rip out the creature’s still beating heart or other vital organ, which then instantly appears in your hand. Blood and other bodily fluids gush out of the creature’s chest. The illusion is perceivable only to you and the target. The target must succeed on a Wisdom saving throw. On a failed Wisdom saving throw, the target believes the illusion is real, causing it to be stunned for the duration. At the end of each of the target’s turns before the spell ends, the target must succeed on a Wisdom saving throw. When it fails its first Wisdom saving throw at the end of its turn, the target becomes unconscious. When it fails the next Wisdom saving throw at the end of its turn, the target drops to 0 hit points and is dying. On a successful Wisdom saving throw, the spell ends. Stunned creatures and unconscious creatures are no longer stunned or unconscious. Dying creatures remain in their current state.
